Kerzner Associates will submit charges to your insurance company. However, we advise that you read your policy carefully, and all that pertains to mental health coverage including deductibles, copayments, maximums, exclusions, etc.
There are fees that your insurance company will not pay and are your responsibility. These may include:
- copays, co-insurance, and deductibles.
- a fee of $85 for an appointment that is missed or is cancelled without 24 hours notice. Insurance companies do not pay for missed appointments. To cancel an appointment, always call our clinicians’ voice mail line at 508-543-2133, option 2. We have 24-hour voice mail for your convenience.
- other miscellaneous charges including but not limited to report writing, phone appointments, record review, school consultations and driving time, which are billed at $150 per hour.
- any fee we submit to your insurance company which is denied.
Kerzner Associates has a “zero balance” policy, and we require that all balances due (e.g., copays, co-insurance, deductibles, missed appointments, etc.) are paid at the time of visit, or in the case of a missed appointment/ late cancellation charge, prior to your next visit. This policy is strictly enforced, and we reserve the right to withhold services without full payment being made.
Kerzner Associates is required to provide your health insurance company with information relevant to the services your clinician provides to you. This includes providing a clinical diagnosis. Sometimes, it may involve clinical information such as treatment plans or summaries. In very rare circumstances, copies of your entire clinical record may need to be provided to your insurance company. This is all done to support the medical necessity of treatment.
In such situations, Kerzner Associates will make every effort to release only the minimum information about you that is necessary for the purpose requested. This information will become part of the insurance company files and will probably be stored electronically. Kerzner Associates will provide you with a copy of any report we submit if you request it. By signing the Patient Services Agreement in this packet of information, you agree that Kerzner Associates can provide requested information to your carrier.
Please note that if a balance is left unpaid, Kerzner Associates has the option to use legal means to secure payment which may mean filing suit in Small Claims Court. This action, although taken very infrequently, requires Kerzner Associates to disclose otherwise confidential information.
